fixture request - QTX relay pack

hi,
it's not a light fixture request but i'm not sure how to make it and thought i'd ask for some help from people that might know, like yourselves.
i am after a fixture for this click here. it's a 12 channel relay pack that i'll be getting to control some lights but there's not fixture for it and i'll be using all 12 channels so wondering if someone could create fixture for me, please.
