Stage Ape Par 64 RGB LED

Hi. I can't seem to create a profile that works correctly for the light in the attached pictures. The awesome part is that the light doesn't have a model number or name on it. All I know is that it's made by Stage Ape. None of the current models on their website have configurations that I've been able to make work. Thanks so much for your help!
